1) download source code from http://frankmancuso.ca/scripts.php
2) unpack and upload to your website.
3) Edit config.php file and enter proper mysql database information.
4) access install.php in your url address
5) Enter the username you would like to use and the password of your chose, plus your email address.
After complete, you should delete your install.php file located on your website. Then point your broswer to login.php and put in the username and password you have selected. If you do not delete install.php the login page will not let you login.
