RepleteSurveys gives you the ability to create surveys with many questions, record survey responses automatically in a MySQL database, and view the results Graphically. Features include:

Installation Instructions

 

  1. Files included in the program:

survey_admin.php, survey.php, survey.cfg, survey_db.sql, survey_db_install.php, DB_class.php

  1. Directory included in the program:

rs_images

  1. Copy these files to your website.
    1. The “rs_images” directory and all files should be located in the same directory.
    2. You may have to upload the “rs_images” directory as “root”.
  2. Edit the file "survey.cfg" to identify your database, yourURL, etc.
  3. To create the DB tables, either, run the file "survey_db.sql" within your DB, or run the program survey_db_install.php from your website: i.e. www.YourDomain.com/survey_db_install.php
  4. Run the adminstration program as: www.yourURL.com/survey_admin.php
  5. To change the appearance of the survey, you have two choices: Globally and Per Survey

 

 I use a style sheet with form settings like "Table_Settings", "Questions_Settings", etc. to control the survey colors, fonts, etc.

    1. Globally

Open the program file "survey_admin.php" and find the function "generate_survey_HTML". In there you will find a style sheet to modify.

    1. Per Survey

When code is generated for each survey, modify the style sheet after you cut&paste the code into your webpage.
